The “Taking Control” Self Management Programme is a structured group workshop:
- Lasting 2.5 hours
- Once a week
- Over 6 weeks
The programme aims to help people with long–term health conditions develop the skills and confidence they need to manage their condition more effectively.
During each session participants look at topics such as:
- Managing symptoms
- Medication
- Working with your health care team
- Setting weekly goals and effective problem solving
- Better communication
- Handling difficult emotions and how to relax
- Tips for eating well
- Safe, easy exercise
By taking part you will learn skills to help manage your health condition as well as developing confidence in yourself. You will also meet other people who share similar experiences and who understand what it’s like to live with a long–term condition. Here is some of the feedback we have received:
