The Stroke Family Liaison Service provides assistance and guidance to families in coping with life after stroke. Our local Stroke Family Support Co–ordinator makes contact with families in hospital or at home.

- Home visits in your local area
- Information on stroke
- Advice on coping with life after stroke
- Secondary prevention
- The setting of goals for recovery
- Emotional support
- Advocacy
- Telephone support
- Link between hospital and home
- Liaison with other health professionals
- Signposting to other services
- Participation in a local carers’ group
